Unix Account Request Form

PBRC Unix accounts may be requested by faculty, students and staff affiliated with the Pacific Biosciences Research Center. In addition, members of the University of Hawaii community who wish to use the Wisconsin Package (GCG) should use this form to request an account for GCG access.

Read the PBRC Acceptable Use Policies before you fill out the form. Requesting a PBRC Unix account implies that you have read and agree to abide by these policies.

After you have submitted the form your account will be created and a sheet will be sent to you with your new account information and password. Failure to fill out the form completely will delay the creation of your account.

Form items marked with asterisk (*) are required.
